ACTION-CARDS Evaluation Form

As an Action-cards instructor you'll use this form to enter data from a training session.

Instruction

You'll need to collect each group of Action-cards evaluation forms, and sum the numbers of "Yes", "No", and "NA's" for all the feedback forms.

E.g. if there are three Evaluation forms for "Evacuation" the max score is 3 x 15 = 45, as there are 15 steps at the Action-cards for Evacuation. Each score is either categorised as "Yes", "No" or "NA".

When you have summed the scores you enter the number in the form below.

Finally you can add the relevant comments that might have been given at each evaluation form in the field "Comments".

It can be advised to do order all the evaluation forms first, then do the counting and calculation and after this start entering the data into the online Action-cards/act scheme which you're looking at right now.
